Situated within the historic Community Hospital in Long Beach, CCCLB brings together a network of community-based partners under one roof to deliver an integrated model of care. It’s a vibrant, working example of what’s possible when we break down silos and put people at the center of care. 

From Street Medicine to Housing Navigation, Behavioral Health Services to recovery programs, every corner of the campus is designed to offer safety, dignity, and opportunity. Now, we’re excited to give you a closer look.
